1. Patient need to request the pin at registration counter which locate both Samitivej Sukhumvit & Samitivej Srinakharin

2. Patient need to apply to be Samitivej Website Member first.

3. Follow the instruction papers which you have been received from our registration to access the my health profile with your hospital number & security pin.
